Intake Process for Vouchers
Once you are selected on the waiting list, the intake process will start. This includes verifying your household members identity, household income, assets, and qualified medical/childcare expenses.
SHA will ask you to upload the following documents during this process, along with additional forms through an online application:
- Social Security Cards for all household members
- Picture ID for all household members 18 and older
- Birth Certificates for all minor household members
- Most recent month of paystubs dated within the last 60 days
- Most recent bank statements dated within the last 60 days
- Social Security benefit letter from the Social Security office dated within the last 60 days
- Four months of child support verification from the Office of Support Enforcement
Once SHA has received this documentation, an intake specialist will review the information, ask for any missing documents, and either approve or deny your application. The timing of this process depends on your responsiveness to SHA’s requests. If you have been approved for the voucher program, the briefing process will be initiated and can be completed online in most cases.
